Memorial Book Program

Are you looking for a special way to honor the memory of a loved one, or to make an anniversary, birthday, graduation, birth, or another event more meaningful?

Consider a book memorial or honoree with a gift of $25 or more, a plaque will be placed in a new book with the name of the honored person and the donor’s name for the life of the book. An acknowledgment letter will be sent to whomever you designate. The book will be selected and purchased by the library.

Forms are available below, or at the library and at most of the local funeral homes.

To make a donation:

• Make your check payable to Bentleyville Public Library
• Drop off or mail your check with the form to 931 Main St., Bentleyville, PA 15314
